Standard Lyme workup: a sensitive ELISA or IFA first, then a Western blot only if the first test is positive or equivocal.

    Order a first-tier EIA/ELISA or IFA against Borrelia burgdorferi antigens.

    If negative and symptoms have lasted under 30 days, consider repeating in 4-6 weeks.

    If positive or equivocal, reflex to a supplemental Western blot.
  4. 04
    Run IgM and IgG blots if symptoms < 30 days; IgG only if symptoms > 30 days.
  5. 05
    Interpret per CDC band criteria; report a positive only when both tiers are positive.
