When telehealth is OK vs in-person

Telehealth is fine for prophylaxis decisions and rash review; in-person is needed for fever, neurologic, or cardiac symptoms.

    Telehealth is appropriate for: deciding on single-dose doxycycline within 72 hours, reviewing photographed rashes, refilling treatment.

    In-person care is needed for: any fever in a child, suspected RMSF, suspected babesiosis, joint swelling needing exam.

    Emergency department: facial palsy, palpitations or syncope, severe headache, ascending weakness, confusion.

    Bring saved tick photos and a symptom log to telehealth visits to compensate for the lack of physical exam.

    If telehealth recommends in-person follow-up, do not delay — same-day or next-day is the norm in tick-borne illness.
